Whenever a covered member or dependent seeks medical treatment (hospital, doctor, clinic, etc.) It is imperative they inform the provider with the correct insurance information. (Example Univera, Blue Cross/Shield of Western New York with your contract number. Univera has new numbers as well as BCBSWNY.
